White boxes indicate agreement between reporting status and assessment based on the case definition. Light grey boxes indicate a potential for discrepancy between reported status and case definition assessment and dark grey boxes indicate that the patient was not reported despite meeting the case definition. ‘Insufficient information to assess’ indicates that there was insufficient assessment to include or exclude a patient based on the case definition. *Potential discrepancy as inclusion may have been based on clinical suspicion
